1. For members currently aged under 18: I am a Parent/Guardian of the musician whose details I have provided in this Permission Form. I give permission for my child to attend the 2025 BB Tour to Japan, including, but not limited to, bus transport, rehearsals, workshops, recreational activities and motel accommodation in multiple share rooms. Or, for members currently aged 18 or over: I will be attending the 2025 BB Tour to Japan. *

2. I/My Child agrees to be bound by the QYO Performer Member Information, Code of Conduct, Terms and Conditions and other QYO Policies, being those existing as at the time of acceptance of me / my child as a QYO member, and thereafter as lawfully modified and amended or added to from time to time. *

3. I hereby release and indemnify QYO, its officers, volunteers, agents and members against any liability in relation to my / my child's participation in QYO activities, and when traveling to and from such activities. *

4. I authorise QYO, in the event of any injury or illness occurring during QYO activities, to obtain on my behalf and at my expense any medical treatment as may be considered appropriate by QYO. This includes headache tablets such as paracetamol as prescribed by the manufacturer. *

5. I am aware of the QYO Privacy Policy available from the QYO website and I consent to the information collected through this form and through QYO activities to be used in accordance with the rights and obligations set out in that policy. *
